PRE-ENTRANCE REQUIREMENTS

NOTES FROM THE HEALTH OFFICE

A few changes have been made to immunization requirements for incoming students in the state of Connecticut.  These changes have been “in the works” for several years, and last week the state finally made them official. These requirements will go into effect for 2011-12.


v     For pre-school students, the requirements are:

 

 DTaP  4 doses
 Polio  3 doses
 *MMR  1 dose on or after 1st birthday
 Hep B  3 doses, last on or after 24 weeks of age
 Varicella 1 dose on or after 1st birthday or disease
 Hib 1 dose on or after 1st birthday
 * Pnuemococcal 1 dase on or after 1st birthday
 *Influenza 1 dose each year between Aug 1 – Dec 31
(2 doses separated by 28 days required for 
those receiving flu shot for first time)

 

 *HepA 2 doses given six months apart. 1st dose on
or after 1st birthday

 

v     For Kindergarten students, the requirements are:

v    DTap

4 doses minimum. Last dose on or after 4th b-day
Polio
3 doses minimum. Last dose on or after 4th b-day
 *MMR 2 doses at least 28 days apart. First dose on/after 1st b-day

 Hep B 3 doses, last dose on or after 24 weeks of age
 Varicella 2 doses, separated by at least 3 months. First dose on/after 1st birthday
or disease verification

 

 Hib 1 dose on/after 1st b-day for children < 5 yrs old
 * Pnuemococcal 1 dose on or after 1st b-day for children < 5 yrs old


v     For students in grades 1-6, the requirements are:

 DTaP 4 doses minimum. Last dose on/after 4th b-day. 
or Td/DTaP  Students starting series at age 7 or older need total of 3 doses.

 

                     
 Polio
3 doses minimum. Last dose on/after 4th b-day.
 *MMR 2 doses at least 28 days apart.  First dose on/after 1st b-day 
 Hep B 3 doses, last dose on/after 24 weeks of age
 Varicella 1 dose on or after 1st b-day or verification of disease
